Either pressed Cruse control by mistake (unlikely) or left it in Drive and foot slipped pressing the throttle hard thinking it was the brake.

With modern cars there are many you leave in 'D' only press the brake pedal to stop 'auto hold' does it's job, no need to touch the brake pedal a  2nd time, 

Touch throttle to restart if it has switched off, or just to proceed if not and engine still running.

Open the door and some do not even switch the engine off, and the car is still in 'D'.

Technology, bloody dangerous with the careless.
