Its worth paying the £10.

I had one a few months ago, and second gear just stopped working.

They wanted to charge me for fixing it... but as I had paid the £10, it was all fine.

Best to take it, as it isn't your car, the owner will want to get any damage repaired, which could well include things that you would put up with on your own car. Also whilst you probably feel confident with your own driving, the car could get hit whilst unattended and no details left or vandalised, in either case you will lose the excess.
