I wouldn't worry any more than any other car.

Some people buy dervs without needing to do big miles. They just like them or don't want to pay a lot at the pumps.

Check the turbo operates OK by using the whole rev range on the test drive, and if that doesn't show up any problems you should be fine unless it has a DPF in which case an Italian Tune-Up may be required...

Given it's just over 3 years old, is it still worth insisting that the garage does a cambelt change if I buy it?

Cambelt will likely be in the region of 60,000 miles or 4 years, which ever comes first, but would do no harm to ask.

a variable vane turbo does need mullering occasionally, my fiat tech says my car has been so reliable (103,000 miles original turbo,clutch,fly,gearbox ect over 3.5 years) is because of how I drive it, he says the old granny approach on the same car over the same time has seen more problems at 30k........

My Octavia VRS was a low mileage one bought back in April with 22K on the clock and three and a half years old, it appealed to me because it doesn't feel at all like an older car. It's amusing the amount of times people ask did I buy it brand new assuming it's only a few months old. Mechanically there's been no problems yet aside from needing its timing belt done at the four year mark, it was done at a Skoda garage who gave it a clean bill of health aside from needing new wiper blades which is not exactly major.
