Would think it's either going to be an injector letting by or a faulty tandem pump.

It's the vacuum/fuel pump driven by the camshaft.

I'd have a look in MBV with diagnostics such as VCDS and see if any of the injectors are reading incorrectly, this should indicate if one is worn or not.
