Bridge inspectios are yearly and will have been completed. Concrete spelling as a structure ages is not uncommon but not really detrimental to the bridge.

That said they should still pay out.

No but it is detrimental to the traffic that flows beneath, hence why the M4 elevated section has netting underneath - to prevent a similar incident on the A4.
