Spare wheel is on a separate hinged frame, which has to be released first, then the boot released. 

Fitting like that would be illegal in EU and much of the rest of the world, as when open the wheel obscures the lights on that side.

Sounds familiar. I think that is why the rear door would not open to 90* on the uncles Rav4.

And the Pajero in Oz which was, to me at least, dangerous as they simply appeared to be fog lights with no other tail lights.

Because the Defender rear door mounted wheel doesn't completely obscure the lights, but one on a remote mounted frame may.
