Cheers mate. No one knows but I'm just saying your thoughts about it being caused by a fault are indeed feesible. I wouldn't dismiss it. "totally smashed" = impact. A crack can indeed be a fault. But actually if you got a replacement and it looks like new, I'd forget it. Seems you were just unlucky however it happened.
