Post Incident Flow Chart

In a previous job I had a 'crash bag' that contained all the relevant paperwork in the sequence I would use them, useful contact numbers/Point Of Contacts, along with an Idiot's Guide flow chart giving a brief breakdown of what to do post incident. 

Just wondering if anyone had seen something similar (or even produced one themselves) that could be printed off and kept in the car in case the unfortunate should happen. 

The one my insurance provider has supplied is a bit basic and vague.
