It's happened once, going up a steep hill in 3rd it's been mapped and floored it.

only ever had it on buses when the cats blocked up so it can happen

when the exhaust is that choked up and it gets a good seeing to then the carbon in the exhaust can catch fire so it defo can happen, used to do it on generators all the time

Sounds like you have a partially plugged cat. Carbon builds up on the pipe immediately in front of the cat. When it comes loose you will get flashes of deep orange.
