Because it's rubber it won't slip.  The jack may leave indentations on the underside of the adaptor but you won't be worried about that.

I have an Arcan XL30R jack with a wide jacking pad which I use with a puck with a slot cut in it.

Cant seem to get them now but the Liftmaster Pro 3 Ton 70mm Ultra Low Profile High Lift Garage Trolley Jack is very similar. No probs with the puck slipping.

Pucks are 3" wide. Are there wider pucks available???

I thought the training pucks were 6" which would spread the load better. Couldn't find specific size on the web though.

Has anyone adapted a puck or something to fit onto an axle stand?? I want to be able to lift the whole front end up for servicing.

I don't see why you couldn't just cut one down to fit the saddle in the axle stand.  Obviously you'll need to jack it somewhere else.
