I got it all from SuperSkoda, I think its Milotec branded with the plate that bolts to the bonnet. Done in ten minutes and that was about 4 years ago.

My Dad sprayed the bracket to match the paint work and I used blue 3M tape to mark the location of the bracket before removing.

Hmm, doesn't look like SuperSkoda sell them anymore either.
