The only thing I can suggest is that you go onto one of the spares request websites and ask on there. Any spares company that has one will then get back to you with a price and availability.
What code is suitable I haven't a clue and to be honest I doubt any of the members here would know either so you might need to talk to a VAG gearbox specialist or even your local Skoda parts people to check.

And to be honest a seized gearbox is rare! I've not heard of one in the 7 years I've been here.
